On this page you will find descriptions of the different forms we use throughout our site. When using these forms, please fill them out as completely as you can before you hit the "SUBMIT" button.

The Information Request Form

You will find this form throughout our site. If you cannot find the exact entertainment you're looking for or you're just not sure of exactly what you need, you can use this form to tell us about your event and the type of entertainment you want.
This is our most widely used form. We understand that entertainment is a very important part of an event. Unfortunately, we cannot list every act that Classique has to offer. Our site would become too large to be effective. So, we give you instead an example of what we can offer. The acts that are listed are just a sample of the quality entertainment that is available through Classique Productions.

The Artist Information Form

This form is designed those of you who wish to submit various Acts and Artists for our review and addition to our site. As you know, more and more people are using the Net these days to shop for just about everything, including entertainment. And, judging by the phone calls we have been getting, entertainers are surfing the Net to find agencies to promote their acts. This form is designed to help those entertainers who are interested in using Classique for just that purpose. Being a full service entertainment agency, we are interested in anything you have to offer (EXCEPT adult entertainment!). The use of this form will greatly help us to speed up the process of contacting you and making you aware of our requirements.

The Comments/Suggestion Form

Because we pride ourselves on quality and service, we want to know if there is anything we can do to make your job easier. This form was designed with that purpose in mind. If there is anything that you feel we are missing; if there is anything that you feel should be corrected (like my spelling!); or, if there are any suggestions you would like to make; or just any comments in general; please feel free to use this form. We welcome any and all comments/suggestions and take each one into serious consideration.
